Please pardon the delay in this reply.  I have been laid up.

I could try to answer your questions from memory but they would probably be unreliable guess work.  But you can get the checked facts from my authorized bibliography, written by Clark and Sanborn some years back, and possibly in your city library.  If not, write to Barrett H. Clark, co-author of the book, address c/o Dramatists Play Service, 6 East 39th Street, New York City, New York.  The reason I cannot look up the facts for you is that all my stuff is in storage, and the bibliography might be in any one of a hundred cases.

Sorry to cause you further delay and trouble but it cannot be helped.
